Welcome to the Child Care Health Consultant Registry!
This Registry is a resource for and about active Child Care Health Consultants (CCHCs) trained through the National Training Center (NTI) or by NTI graduates. By completing and posting this form, you can help child care providers in your area find the consultation support they need to meet the specific needs of the children and families they serve. National and state leaders will also use the information provided by you and others to get a clear picture about the CCHC resources in your state and across the country
Please complete all required fields. At the end of the form, you will find an area for additional comments. Please use that space to describe aspects of your CCHC work that are not covered by the other Registry fields. And remember, you can return at any time to your secure record to update it or remove it from the Registry.
After we approve your registration, we will send you a confirmation e-mail with your password. You can use this password to update or remove your information at any time.
To help your registration go smoothly, keep these points in mind:
- While completing your Registry entry, do not click on your browser’s "back" arrow. Clicking the "back" arrow may cause you to lose information you have completed, and you may have to re-enter it. You will have an opportunity to review and edit your information once you complete the entire form.
- Be sure to complete all of the required fields (these are marked with an asterisk). You will not be able to continue until you fill in all of the required fields
- When you are finished, please do not click "Submit" more than once.
